Katie O’ Events is thrilled to coordinate the 23rd Annual Dr. Pamela J. Harper New Jerusalem Home of the Saved Church Community Block Party, in collaboration with CDPHP® Healthy Neighborhood!
Please join us on Saturday, August 22 from 11 a.m. - 2 p.m. at the 100th block of Sheridan Ave in Albany.
Your organization is invited to participate in the 23rd Annual Dr. Pamela J. Harper New Jerusalem Home of the Saved Church Community Block Party to help promote a strong sense of community and good health. This event is a perfect opportunity to connect the community to the important products, programs, care, and services provided by your organization.
We anticipate great interest in the entertainment and activities planned for this event, including live music, family activities, and free food.
The event will be held outdoors, rain or shine. If you’d like to participate, please reserve your spot by completing the following steps:

Fill out and submit the vendor application by Friday, August 7, 2026.

Provide a certificate of insurance listing Wellness Way – 6 Wellness Way, Latham, NY 12110 as the certificate holder and additional insured.
To help ensure all proceeds from this event go towards supporting the community, we are asking vendors to bring their own table or cover the cost of the table rental ($30). Please see below for payment options.

COI REQUEST INFORMATION
At a minimum, each HIGH-RISK Vendor should have the following Insurance:

Commercial General Liability insurance written on an Occurrence basis with limits of $1,000,000 Each Occurrence and $2,000,000 Annual Aggregate naming < CDPHP at 6 Wellness Way, Latham, NY 12110 > as an Additional Insured on a primary and noncontributory basis.
